"Each time she put her hands back on the wheel, she took them back off the wheel after a few seconds", Tesla's report, as released by South Jordan Police Department, said.
According to Tesla technicians, the recovered data from the vehicle showed that the driver repeatedly cancelled and re-engaged the Autopilot features.
At the time, she told the police that the vehicle was on Autopilot and later admitted that she was on her phone.
Police say the auto smashed into a stationary firetruck at around 100km/h, leaving the driver with a broken right ankle.
Drivers are repeatedly advised Autopilot features do not make Tesla vehicles "autonomous" and that the driver absolutely must remain vigilant with their eyes on theroad, hands on the wheel and they must be prepared to take any and all action necessary to avoid hazards on the road.
"Tesla has always been clear that Autopilot doesn't make the auto impervious to all accidents", the statement said.
The driver of the firetruck was checked for whiplash injuries but did not go to the hospital.
During the driver's trip, she reportedly toggled Autopilot and active cruise control on and off on multiple occasions, and modified the vehicle's cruise speed at varying points.
